Library Cards
To register for a Mid York Library Card, a patron must live, work, or go to school in Herkimer, Madison, or Oneida County. The card may be used at all 43 Mid York Libraries located in Herkimer, Oneida and Madison counties. Registration forms will need to be completed and identification is required as well as proof of current address. To ensure there are no duplicate accounts, staff is responsible to verify the potential borrower does not have an existing library record. A library card might not be issued right away if further verification is needed or a decision about the application is needed from the Library Director.
Children between ages five 5 and 13 may obtain a library card requiring the signature and ID of a parent or guardian. A library card will be issued to adult borrowers (age 14 and over) who has valid identification listing a current address. Forms of acceptable ID are as followed: driver’s license or permit, passport, car registration, property deed, benefits card, tax bill, current telephone or utility bill, notarized affidavit, state ID card, lease agreement/rent receipt, and a check/checkbook with name and address. Adult borrowers under the age of 18 without identification may use school IDs.
All patrons are limited to taking out two items the first time they use their new library card. Once the items are returned the patron may increase the number of items.
The cost of a replacement card is $1.

Staff of the Frank J. Basloe Library may offer to take a patron’s photo to be used in the library records only for confirmation of identity within the library system. A properly executed warrant or the written permission of the patron or legal guardian is required for the release of the photo to any organization outside the library system itself. Patrons may refuse to be photographed if they do not want their photo on their record.
Borrowing library materials is a privilege, not a right. Library cards should be presented when borrowing materials. Patrons are to use their own library card. Patrons may not use another person’s library card. Valid ID in lieu of presenting a library card is acceptable.

Borrowing Materials
Borrowing privileges extend to all library materials and will only be revoked if the patron refuses to return overdue materials or accrues excessive unpaid fines. If a patron’s fine reaches $5.00 or more, their borrowing and computer use privileges will be blocked.
Patron accounts have a limit of 40 items checked out at one time. This includes all materials. The only items with checkout limits are DVDs. Adults can check out 10 DVDs and children can check out 3 DVDs at one time.
Fines and Fees
The Frank J. Basloe Library charges fines for overdue materials. The fines per day are as followed:
Books, Audiobooks, and Magazines – 15 cents per day late
DVDs – $2.00 per day late
Fines can be paid in person with cash, or online through the Mid York catalog.
Lost Materials
Items that are considered “lost” are items that have not been returned and are long overdue. Patrons will be charged the full amount of the lost item. Lost items fines can be removed by paying the fee, returning the lost item, or replacing the lost item. Patrons are still responsible for overdue fines related to the lost item.
